Epiphany Grove Client Agreement
This Agreement is entered into as of the date of electronic signature by and between:
Coach/Practitioner:
Brandy Lee / Epiphany Grove LLC (hereinafter “Coach/Practitioner”)
Client:
The individual who completes and electronically signs this Agreement (hereinafter “Client”).
WHEREAS, the Coach/Practitioner offers specialized services, and the Client wishes to engage in these services, the parties agree as follows:
1. Purpose
The purpose of this Agreement is to establish the terms and conditions under which the Coach/Practitioner will provide health & wellness coaching, life coaching, and Reiki services to the Client. These services are intended to support the Client’s personal development, well-being, and goals in a holistic and collaborative manner.
2. Nature of Services
Coaching (Health & Wellness + Life Coaching)
Coaching is a collaborative, client-driven process focused on goal-setting, mindset development, personal growth, and lifestyle guidance.
Coaching is not therapy, counseling, medical treatment, legal advice, or financial advice.
The Client understands that the Coach does not diagnose conditions, prescribe treatments, or guarantee outcomes.
Reiki
Reiki is a complementary, energy-based wellness practice that supports relaxation, stress reduction, and balance.
Reiki is not a substitute for medical or psychological care.
Reiki practitioners do not diagnose conditions, prescribe treatments, or interfere with medical care.
The Client acknowledges that the Coach/Practitioner does not provide medical, psychological, legal, or financial advice, and any decisions made by the Client are made voluntarily and at the Client’s own discretion.
3. Services Provided
The Coach/Practitioner shall provide the specific services, session types, format, frequency, and duration selected by the Client and outlined in Section 10 (Client Service Order).
Unless otherwise specified in Section 10, coaching packages may include:
A one-time 60-minute onboarding session in the first month
Secure client portal access
Between-session messaging support
Personalized wellness practices and tools
All services are defined solely by this Agreement and Section 10 (Client Service Order).
4. Fees & Payment
The Client agrees to pay the Total Fee, utilizing the Payment Method, and according to the Due Dates or Schedule specified in Section 10 (Client Service Order).
5. Payment, Refund & Scheduling Policies
Payments
Payment is due as outlined in Section 10 (Client Service Order).
Payment must be completed prior to the first session unless an installment plan has been explicitly agreed upon and documented.
The Client agrees not to initiate a chargeback or payment dispute without first providing written notice and allowing a reasonable opportunity to resolve the matter directly with the Coach/Practitioner.
Refund Policy
All fees paid for services, packages, memberships, and single sessions are non-refundable.
Due to the immediate allocation of resources, scheduling of the Coach/Practitioner’s time, prior access to a free Discovery Session, and delivery of proprietary materials, the Client acknowledges that all sales are final, regardless of whether the Client completes all sessions or terminates this Agreement early.
Rescheduling & Missed Sessions
Cancellations require at least 24 hours’ notice to reschedule.
Sessions canceled with less than 24 hours’ notice may be forfeited.
Missed sessions without notice may be forfeited and will not be refunded or rescheduled.
Expiration
Unless otherwise specified in Section 10, all purchased sessions must be completed within six (6) months of purchase.
Expired sessions are forfeited and will not be extended unless agreed to in writing.
6. Confidentiality
All information shared by the Client during sessions will be kept confidential, except where disclosure is required by law (such as in response to a valid court order).
The Coach/Practitioner may use generalized or anonymized experiences for professional development, education, or marketing purposes, provided no identifying information is disclosed.
7. Client Responsibilities
The Client agrees:
To be proactive, honest, and engaged during sessions.
To communicate openly about needs, boundaries, and concerns.
To understand that results depend on personal effort, decisions, and follow-through.
To seek appropriate licensed medical or mental health support for medical or psychological concerns.
The Client acknowledges that services are supportive in nature and are not curative or diagnostic.
8. Termination
Either party may terminate this Agreement with written notice.
In the event of termination:
Termination does not relieve the Client of financial obligations already incurred.
9. Liability Disclaimer & Assumption of Risk
The Client acknowledges that participation in coaching and Reiki services may involve personal reflection, emotional exploration, lifestyle adjustments, and physical relaxation responses.
The Client voluntarily assumes all risks associated with participation.
To the fullest extent permitted by law:
Epiphany Grove LLC and the Coach/Practitioner shall not be liable for any direct, indirect, incidental, special, or consequential damages arising from the Client’s participation in services.
Any claim for damages shall be limited to the total amount paid by the Client to the Coach/Practitioner in the thirty (30) days preceding the claim.
The Client understands they are solely responsible for their decisions, actions, and results.
